On XXXXX the Tax Commission issued an order revoking license No. XXXXX for failure to comply with the laws of the State of Utah. By Memorandum dated XXXXX the Collection Division requested that the license be reinstated on the grounds that the tax has been paid and arrangements have been made for the payment of penalty and interest. These arrangements require at least $$$$$ a month to be paid to the Tax Commission towards the penalties of interest with a trust account being established and with weekly deposits being made to that trust account for current accruing liabilities.

DECISION AND ORDER

Based upon the forgoing, the Tax Commission reinstates license No. XXXXX on the terms and conditions that the Respondent make monthly payments in the sum of $$$$$ until the penalty and interest has been paid and that a trust account be established in which weekly deposits are made of the sales taxes collected during that week. Finally, that all quarterly reports are timely and properly submitted including payment. It is so ordered.
